Mobile Tent Program Loan Request Form

Submission of a Mobile Tent Program Loan Request Form constitutes your and the Organization’s agreement to the following Loan Agreement Conditions and Liability Release Terms which follow. By submitting this Request Form, the Renting Organization agrees to these Terms and Conditions.

Loan Agreement Conditions
The person submitting the Mobile Tent Program Loan Request Form is authorized by the Organization to make the request and to agree to these Conditions and Terms. The Organization agrees to:
• Review set up instructions and training videos in advance to use.
• Return the tent(s) in their original condition. This includes disinfecting all items in the kit, including all surfaces of the tent(s) and boxes, with a non-bleach cleaner; manufacturer recommends Simple Green products or similar.
• Report any product issues or damages immediately to Lane Community Health Council.
• If gasoline is used, fill up the generator and gas can to the levels at initial pick-up.
• Return the tent by the agreed upon date.

Liability Release Terms
Organizations who participate in the Mobile Tent Program with Lane Community Health Council acknowledge that there are certain risks associated with the use of the product(s) which cannot be entirely mitigated or avoided despite the exercise of caution, including, without limitation, the risk of personal injury, death, and property damage. The Organization agrees to indemnify, defend, and hold harmless Lane Community Health Council, its directors, officers, employees, volunteers and agents (collectively, “LCHC and Agents”) from all claims, losses, expenses, damages, liabilities, injuries, and causes of action of any nature, including, but not limited to, personal injury, wrongful death, and property damage (collectively, “Claims”), that may in any manner arise from or be related to the use, installation, and/or possession of the product(s) borrowed from Lane Community Health Council, and the Organization releases LCHC and Agents from any Claims Organization may have associated with the use, installation and/or possession of such product(s).
